An Out of Home Relative child care provider is one who:
- Provides care for a relative’s child outside of that child’s home.
- Must be at least 18 years of age and a grandparent, great grandparent, sibling, or aunt/uncle of the child.
- Is related through marriage, blood relationship or court decree. You may be asked to verify your relationship to the child.
- Cares for 6 or fewer related children under 16 years of age (including the provider’s own children), and provides care for less than 24 hours a day.
- Must have a referral from an DCF worker to enroll as an Out of Home Relative provider.
To enroll as an Out of Home Relative Child Care Provider you will need to:
- Have been referred to DCF Provider Enrollment staff by an DCF worker to provide Out of Home Relative child care.
- Gather your information:
- Your social security number or EIN (Employer Identification Number)
- The name and social security number of the parent whose child(ren) you will be providing care
- The name(s) of the child(ren) you will be providing care
- The name, aliases, date of birth, social security number of all affiliates. An affiliate is anyone (including you) age 10 or older who resides, works, or volunteers in a child care home.
- Download the DCF Provider Handbook. You must read and agree to its contents. Download the handbook.
- Complete the Online Enrollment forms OR Use the forms included in the Handbook to complete on paper.
- If you are unable to print the handbook, you can contact your local DCF for a copy.
- You must submit the following completed documents with your application. You can find these documents in the DCF Child Care Provider Handbook.

- Policy Statement on Discipline
After completing your online enrollment, you may return to the Provider Portal. You will have the opportunity to attach scanned copies of these documents. If you do not have documents in an electronic format, you can mail the documents to the DCF office listed on your application. Please note: Mailing required documentation may delay processing of the application.
